The Urban County Program requests approval of Subrecipient Agreement with Easter Seals Rio Grande Valley (Health Services) in the amount of $7,500.00 for Urban County Program Year 23 (2010).
BACKGROUND
All funding will be utilized for the reimbursement of outpatient rehabilitation services . The Agreement shall be effective on July 6, 2010 and expire May 30, 2011. (See the attached Exhibits).
