BACKGROUND

1.  Applicable NIGP code : 961-17;
2.  Cost:  $125.00/hr. w/min. of 2 hours;
     Min. 50 devices provided under said charge;
3.  Initial term:  Effective 11-20-18 unless otherwise changed by both DD#1& CC through 12-31-2019;
4.  2- Additional 1 year terms under same rates, terms and conditions.
5.  Both HCDD31 and HC will be sharing cost of services equally
