CAPTION
1. Requesting authorization to accept the DSHS Texas Cardiovascular Disease and Stroke mini-grant in the amount of $5,000.00. The Cardiovascular Disease and Stroke program will reimburse Hidalgo County up to $5,000.00 for creating a work plan to develop a stroke awareness media and community outreach campaign that will be targeted to the citizens of Hidalgo County and for providing a report to DSHS that will include an evaluation of the work plan.
2. Requesting authorization to initiate and implement the attached Hidalgo County Cardiovascular Disease and Stroke work plan. The work plan will start on June 21, 2011 and run through August 31, 2011.
3. Requesting approval of the attached Cardiovascular Disease and Stroke budget amendment in the amount of $5,000.00
4. Requesting approval of the Certificate of Revenue as certified by the County Auditor.

BACKGROUND
DSHS is treating this program as an unencumbered, non-purchase order claim; therefore, it requires no grant application on County’s part and there will be no grant contract issued. Submittal of the work plan, evaluation report and invoice is what is required.
