a.  Presentation of the scoring grid (for purposes of CC determination as qualified so as to proceed with next step of the procurement process) of the sole response received (as detailed in documentation attached herein) for: Chief Physician Services for Hidalgo County Health and Human Services Department-RFQ No.: 2011-138-07-20CGA;

b. Authority for the Purchasing Department &/or other county staff to negotiate a professional services contract with the qualified participant _____________________ for the provision of: "Chief Physician Services" for Hidalgo County Health and Human Services Department.
