Requesting authorization to accept the attached Cardiovascular Health and Wellness Program grant contract #2011-039140-001 in the amount of $5,000.00.
BACKGROUND
This program was accepted by CC on 06/21/11 through AI-27129. A Certification of Revenue was also approved on the same day. The Texas Department of State Health Services initially treated this program as an unencumbered, non-purchase order claim which would require no grant contract; Submittal of a work plan and an evaluation report to them followed by an invoice is what was required to reimburse Hidalgo County the $5,000.00.
DSHS has reconsidered the method of delivery for these monies and is now requiring that a grant contract be processed; hence, this agenda item and the grant contract that is being presented to CC at this time.
DSHS staff have indicated they will honor the expenditures incurred within the initial work plan period of June 21, 2011 through August 31, 2011.
