BACKGROUND
On 7/23/2024 (AI-960015) we requested approval to submit CMS Form 116. CMS 116 is the College of American Pathologist (CAP) application. On 12/17/2024, our CLIA application was approved, and they are requesting that we submit the attached Form 3225.
