BACKGROUND

Proposed project is to include leveling and roof repairs but not limited to any other accessories as identified and made part of on Addendum #1 at walk thru held on May 18, 2008 for proposed project attached is the approval from Texas Department of State Health Services.

Funding for propsoed project has been reviewed by our PO Administrator and available for funding under acct. #8-1292-441-00-350-001-8-431.
