Discuss, consider, and take appropriate action on approving the Symetra Implementation Documents for RFP #25RFP47 Employer Group Life Insurance, AD&D, Voluntary Life Insurance, Voluntary Short-Term Disability Insurance, Voluntary Long-Term Disability Insurance, Voluntary Accident Insurance, Voluntary Critical Illness Insurance for Benefits and authorizing the execution of the agreements.

Background

These are the Implementation Documents associated with the previous award of RFP #25RFP47. The Symetra implementation documents (Master Application, Expense Reimbursement Agreement, Customer Electronic Consent and Disclosure Forms are a requirement by Symetra to complete our benefit set up specifically for the 2026 award to Symetra for the Accident and Critical Illness Voluntary Benefit plans.  Additionally, this will enable the required electronic file feed that will be transmitted between Benefitfocus our enrollment portal vendor and Symetra. This cost for the Benefitfocus file feed of $3,500 will be offset and allows us to utilize Symetra’s Expense Reimbursement Form to remit the implementation cost of the file feed as provided in the RFP proposal submitted by Symetra. Contract Audit and General Counsel reviewed. The funding source is 01.0885.0885.004058 and the point of contact is Shelley Loughery.
