Discuss, consider, and take appropriate action on approving a MOU between Williamson County Emergency Medical Services and WellMed and authorizing the County Judge to sign the same and any necessary related documents.

Williamson County Emergency Medical Services is entering into a 12-month pilot program (MOU) with WellMed Medical Management designed to strengthen local healthcare access and reduce taxpayer costs through a partnership between the WCEMS Community Health Paramedic team and WellMed Medical Management.  This pilot integrates Community Health Paramedicine, a proven, evidence-based model for transitional and preventative care, with WellMed’s care coordination framework. Community Paramedics provide proactive in-home visits, follow-up assessments, medication reconciliation, and health education for high-risk, recently discharged, or frequently-employed patients.
