Reason for Recommended Action/Background
The Public Health Branch has completed its 2023-25 Community Health Assessment (CHA). This report identifies the 11 priorities Significant Health Needs (SHNs) that were identified in partnership with the hospitals and healthcare systems through a systematic data analysis process. Additionally the CHA include data on disparities in the social determinants of health among various groups and geographic locations within the county. The CHA is also a tool for action to be used by partners, The County, and others to help direct efforts and resources to help improve health outcomes in our community.
The CHA is a tool to inform strategies in the Community Health Improvement Plan (CHIP), which is currently in development. The CHIP is a roadmap for improving health outcomes in the selected priorities that partners and collaborators have agreed to work on over the next 3 years. The CHIP will be complete by June 2023. To be fully successful, the CHIP strategies will require a multi-sectoral partnership across all leadership levels in Yolo County, including support from County leadership. This presentation will be an update to the Board on the Significant Health Needs as well as the focus areas of the CHIP at this time.
