Delegate signing authority to the Yolo County Procurement Manager to execute the agreement with Granite Wellness in an amount of $237,500 for the period of January 1, 2024 through June 30, 2024 for the provision of Substance Use Disorder Adult Continuum of Care services (No general fund impact) (Sullivan)

Recommended Action

  1. Delegate signing authority to the Yolo County Procurement Manager to execute the agreement with Granite Wellness in an amount of $237,500 for the period of January 1, 2024 through June 30, 2024 for the provision of Substance Use Disorder Adult Continuum of Care services; 
     
  2. Delegate authority to the Director of the Yolo County Health and Human Services Agency or her/his designee to 1) exercise optional year extensions of up to a maximum five (5) year contract term; and 2) issue related option year notices.

Reason for Recommended Action/Background

The County entered into an agreement with the California Department of Health Care Services (DHCS) to provide Drug Medi-Cal Organized Delivery System (DMC-ODS) Substance Use Disorder (SUD) treatment services to eligible clients. DMC-ODS is a program for organized SUD services to Medi-Cal eligible individuals with SUD residing in a county that has elected to participate in the DMC-ODS. The program provides beneficiaries with access to care and system interaction needed to achieve sustainable recovery. The County also receives funding from a variety of other federal and state funding streams that assist the County in provided SUD treatment services to various client populations, including but not limited to:  Substance Abuse Prevention and Treatment (SABG) Block Grant Funds; Substance Abuse and Mental Health Services Administration (SAMHSA); CalWORKS and CWS funding; 2011 Realignment, Opioid Settlement Funds and Intergovernmental Transfer (IGT) Funds.
SUD Continuum of Cares services includes the following:
  • Residential treatment services
  • Outpatient and intensive outpatient treatment services
  • Residential withdrawal management (detoxification) services
  • Care Coordination
  • Recovery services
  • Clinician consultation services
  • Recovery Residence
  • Narcotic treatment program services.
These services will be provided to any eligible Yolo County resident referred through an access point established by Yolo County or directly requesting services through a provider and will include, but not be limited to, the following populations/programs:
  • CalWORKS Participants
  • Individuals with an open Child Welfare Services (CWS) Case Plan
  • Individuals who are Pregnant or Postpartum (Perinatal Services)
  • Individuals who are involved with the Criminal Justice and those partially funded by system (AB109)
  • Medically Indigent Adults
  • Individuals with Co-Occurring substance use and mental health disorders
Granite Wellness will provide withdrawal management, low intensity residential, high intensity residential, perinatal residential services, care coordination, and clinician consultation services.

County Counsel has approved this Agreement as to form.

Competitive Bid Process/Vendor Performance

On October 13,2023, the then acting Yolo County Procurement Manager approved a sole source procurement for these services.

A sole source was deemed appropriate for these services because the County has a contractual obligation with the State to provide SUD continuum of Care services that include 3.1 residential, withdrawal management 3.2, residential 3.5 and perinatal services. Aside from Granite Bay there is only one other provider that can provide 3.1 and 3.5 residential services and Granite Bay is the only provider that has the capacity to provide perinatal residential services.

By contracting these services with Granite Bay, the County is able to meet the state requirements of providing SUD services to all levels of care in a timely manner. The County will also meet the need of Yolo County Medi-Cal members for residential 3.1, residential 3.5, and residential perinatal services. The provider costs for these services align with County SUD budgets and set rates.

Approval of the request herein will enable HHSA to expedite the approval of the agreement for these crucial services and prevent lapse in services. The agreement is currently with Granite Wellness for their review and signature. Once HHSA has obtained their approval and signature, the agreement will be routed to the Procurement Manager for the final County signature.

No general funds are required by this action. These services will be funded by Realignment and Federal Financial Participation (FFP) Medi-Cal eligible services drawdown. The total amount of this action is $237,500 for the period of January 1, 2024 through June 30,2024. The amount of $237,500 is included in the HHSA adopted budget for fiscal year 2023-24. The related funding will be included in the requested budget process for future fiscal years.

In no event shall the term of the Agreement extend beyond December 31, 2028, nor shall the total contract maximum exceed the amount of TWO MILLION THREE HUNDRED SEVENTY-FIVE THOUSAND DOLLARS ($2,375,000), unless otherwise agreed to in writing by the parties and in conformity with the then-current County Procurement Policy.
