Approve and authorize the Chair to sign the first amendment to State Agreement No. 21-10600 with California Department of Public Health (CDPH) to extend the agreement through January 31, 2026 and reduce funding by $256,112 for a new total grant amount of $384,168, for the provision of implementing the Disease Intervention Specialist (DIS) workforce grant. Recommended Action
- Approve and authorize the Chair to sign the first amendment to State Agreement No. 21-10600 with California Department of Public Health (CDPH) to extend the agreement through January 31, 2026 and reduce funding by $256,112 for a new total grant amount of $384,168, for the provision of implementing the Disease Intervention Specialist (DIS) workforce grant;
- Authorize Yolo County Procurement Manager, or designee, to execute any non-financial extensions or amendments of the Agreement, upon approval as to form by the County Counsel; and
- Authorize the Director of Health and Human Services Agency, or designee, to execute any fiscal and program reports required under this Agreement, including reports required under any extensions or amendments thereto.

Reason for Recommended Action/Background
The California Department of Public Health (CDPH), Sexually Transmitted Disease Control Branch (STDCB) was awarded Disease Intervention Specialists (DIS) Workforce Development funding as part of the American Rescue Plan Act of 2021 to address 21st century outbreak response.
This funding has allowed Yolo County Health and Human Services Agency (HHSA) to expand and enhance frontline public health staff; conduct DIS workforce training and skills building; build organizational capacity for outbreak response; and evaluate and improve recruitment, training, and outbreak response efforts that address sexually transmitted disease (STD), HIV, COVID-19, and other infectious disease prevention and response needs of Yolo County.
This amendment changes the agreement’s termination date from December 31, 2025 to January 31, 2026 and removes the remaining unobligated funds for years 4 and 5. HHSA will be able to spend any unused funding from Years 1-3 through January 31, 2026.
