Subject

Authorize the Director of Health and Human Services Agency, or designee, to execute an agreement with Sierra Health Foundation in the amount of $134,853.40 for the period of July 1, 2024 through June 30, 2025 for the provision of Foster Youth Substance Use Disorder Project. (No general fund impact) (Sullivan)

Recommended Action

  1. Authorize the Director of Health and Human Services Agency, or designee, to execute an agreement with Sierra Health Foundation in the amount of $134,853.40 for the period of July 1, 2024 through June 30, 2025 for the provision of Foster Youth Substance Use Disorder Project; and
     
  2. Delegate authority to the Yolo County Health and Human Services Agency Director, or designee, to execute any amendments, including financial amendments and non-financial amendments, upon approval as to form by County Counsel.

Reason for Recommended Action/Background

Sierra Health Foundation entered into State Agreement No. 23-30301 dated July 1,2023 for the Foster Substance Use Disorder Project (the Project) where they agreed to develop and implement the Foster Youth Substance Use Disorder Evidence-Based and Promising Practices Program to provide evidence-based models and promising practices to serve foster youth with substance use disorders including those who are residing in family-based settings.

Yolo County Health and Human Services Agency (HHSA) is a subcontractor for these funds. HHSA will establish models and practices that include trauma-informed approaches to serving foster youth, harm-reduction approaches in services delivery, post-treatment support planning, and training for clinical services providers to support foster youth with co-occurring substance use and mental health needs.

This is a revenue agreement and does not contain County performance measures.
