Recommended Action
Approve first amendment to Agreement No. 24-252 with Yolo Community Care Continuum (YCCC) to add funding in the amount of $57,000 for a new contract maximum of $2,307,000 for the period of July 1, 2024 through June 30, 2027; and update County contacts and email addresses, for the provision of adult residential services through the Farmhouse Residential Treatment Program.

Reason for Recommended Action/Background
Farmhouse is a long-term residential treatment program (Farmhouse Program) provided by Yolo Community Care Continuum (YCCC) as a therapeutic residential setting that provides services 24 hours a day, 7 days a week with a full range of social rehabilitation services for individuals diagnosed with a severe mental illness (SMI) who require intensive support to avoid hospitalization or institutionalization.
YCCC’s Farmhouse Program is a DHCS licensed Adult Residential Facility and the only long-term adult residential treatment program available for SMI adults in Yolo County. The Farmhouse Program provides a full range of social rehabilitation services including:

- Individual group counseling
- Crisis intervention
- Planned activities
- Counseling with family, as applicable
- Development of Community support systems for clients
- Transportation to necessary medical/community appointments
- Client advocacy
- Socialization activities in the community
- Pre-vocational and vocation services
- Program length and date of discharge based on client’s needs.
On or about September 10, 2024, the County entered into an Agreement with Yolo Community Care Continuum to provide Adult Residential services through the Farmhouse Program with an initial term of July 1, 2024 through June 2027.
Now, HHSA requests approval of this amendment to add funding in the amount of $57,000 for fiscal year 2024-25. The additional funding is needed to cover the increased client cost. Based on the last spend down report, the program is projected to overspend the contract amount.

Competitive Bid Process/Vendor Performance
Competitive Bid Process
On May 3, 2024, the County’s Deputy Director of GSD/Manager of Procurement/County Purchasing Agent (Purchasing Agent) approved a sole/single source procurement for these services, based on satisfactory service/reasonable prices and to avoid the interruption of County business.
Without sole/single source approval for these services, the County would be negatively impacted, including losing 10 dedicated beds for Yolo County clients and a local long-term adult residential treatment facility. Factors considered in the sole source request, included but were not limited to 1) obtaining local long-term residential treatment options for clients that offer both residential and pre-vocational programs; 2) competitive pricing and long-term cost savings keeping clients out of higher priced facilities like a Psychiatric Health Facility (PHF); and 3) ease of geographical access as this facility is 7 miles northwest of Davis, CA. Often a primary treatment goal is to keep clients in residential placements that are close to home, rather than placements in other counties.
Vendor Performance
HHSA confirms Yolo Community Care Continuum is performing satisfactorily under this Agreement. Performance Measures were originally included in this Agreement Performance Measures and are not being revised at this time (See Att. B. Performance Measures).
Farmhouse provided services to eleven (11) Yolo County beneficiaries during the first half of this FY 2024-25. One hundred percent (100%) of the clients demonstrated engagement with the services offered. None of the clients admitted were hospitalized ,or incarcerated during the treatment period. Also, none of their clients had psychiatric re-admission to the inpatient hospitals within six (6) months.

No general funds are required by this action. These services will be funded by Realignment. The action increases the contract maximum by $57,000 for a total of $2,307,000 for the period of July 1, 2024 through June 30, 2027. The amount of $57,000 is included in the HHSA adopted budget for fiscal year 2024-25. The related funding will be included in the requested budget process for future fiscal years.

In no event shall the term of the Agreement extend beyond June 30, 2029, nor shall the total contract maximum exceed the amount of THREE MILLION EIGHT HUNDRED SEVEN THOUSAND DOLLARS ($3,807,000), unless otherwise agreed to in writing by the Parties and in conformity with the then-current Yolo County Procurement Policy approved by the Yolo County Board of Supervisors.
