Recommended Action
Approve third amendment to Agreement No. 23-320 with Yolo Community Care Continuum to add funding in the amount of $120,000 by adding $60,000 for 2024-25 and $60,000 for 2025-26 for a new contract maximum of $3,395,667.79 for the period of January 1, 2024 through June 30, 2026, for the provision of Adult Crisis Residential Services at Safe Harbor.

Reason for Recommended Action/Background
Crisis Residential Treatment Services are therapeutic or rehabilitative services in a non-institutional residential setting provided as a structured program as an alternative to hospitalization for beneficiaries experiencing an acute psychiatric episode or crisis, who do not have medical complications requiring skilled nursing care. The services include a range of activities and services that support beneficiaries in their efforts to restore, maintain, and apply interpersonal and independent living skills, and to access community support systems. The services are available 24 hours a day, seven days a week. Service activities may include but are not limited to assessment, plan development, therapy, rehabilitation, collateral, and crisis intervention. HHSA expanded the breadth of Crisis Residential Treatment in Yolo County by adding Short-Term beds for stays of up to 4 days for intensive crisis de-escalation services to the existing Longer-Term beds (for stays up to 30 days) for clients in need of prolonged interventions. This is part of HHSA’s efforts to adopt a version of the Crisis Now model.
On or about December 8, 2023, the County entered into Agreement No. 23-320 (the Agreement) with Yolo Community Care Continuum for the provision of adult crisis residential services at Safe Harbor for the period of January 1, 2024 through June 30, 2026. The Agreement included 2 optional extensions for Fiscal Years 2026-27 and 2027-28.
On or about August 24, 2024, the Parties amended the Agreement via the first amendment to specify funding sources, updated County email addresses and add Exhibit F- Safe Harbor Triage Checklist. On or about October 8, 2024, the Parties further amended the Agreement via the second amendment to add additional MHSA funding for 2023-24 to cover remaining year end claims.
Approval of this third amendment will add additional Realignment funding for 2024-25 and 2025-26. Based on overall utilization (short-term and long-term bed usage) the program is projected to overspend the current contract due to increase usage of the services, therefore we are amending the contract to cover the cost of client service.

Competitive Bid Process
On June 28, 2023, the County via the Department of General Services, Procurement Division (DGS-Procurement) issued a Request for Proposal (RFP) on behalf of the Health and Human Services Agency (HHSA) for Crisis Residential Services. The solicitation was open for thirty-four (34) days, (just under 5 weeks). On July 11, 2023, DGS and HHSA offered potential bidders an opportunity to ask questions during a bidder’s conference.
The County received one (1) proposal from this solicitation. DGS reviewed the proposal for minimum requirements and moved it forward to the Review Committee, comprised of five (5) evaluators, four (4) County Staff and one (1) from outside of the County. The Review Committee evaluated the proposal and determined that the proposal submitted by Yolo Community Care Continuum met all the required qualifications necessary to successfully provide these services. On September 18, 2023, the County issued an Intent to Award letter.

Vendor Performance
Performance Measures were originally included in the underlying Agreement and are not being revised at this time (See Att. B. Performance Measures).
HHSA confirms Yolo Community Care Continuum is performing satisfactorily under this Agreement. During their stay at Safe Harbor Crisis Treatment program, eighty-nine percent (89%) of clients demonstrated therapeutic program engagement and ninety percent (90%) of clients met their individual short-term goals during their stay. On average, all clients served stabilized within twenty-four (24) days and ninety-two percent (92%) of clients served during first half of FY 2024/25 did not evidence the need for psychiatric hospitalization within six (6) months after a Safe Harbor stay.

No general funds are required by this action. These services will be funded by Realignment. The action increases the contract maximum by $120,000 for a total of $3,395,667.79 for the period of January 1, 2024 through June 30, 2026. The amount of $60,000 is included in the HHSA adopted budget for fiscal year 2024-25. The amount of $60,000 will be included in HHSA proposed budget for fiscal year 2025-26. The related funding will be included in the requested budget process for future fiscal years.
