Recommended Action
Approve agreement with Vista Pacifica Enterprises, Inc. in the amount of $3,140,676 for the period of July 1, 2025 through June 30, 2028 for the provision of 24-hour supervised clinical and nursing care services to long-term consumers with severe mental illness (SMI).

Reason for Recommended Action/Background
As the local Mental Health Plan (MHP) for Specialty Mental Health Services, the County is mandated to provide consistent and quality care in secure residential settings, when deemed necessary, for the safety and stability of individuals with SMI.
Vista Pacifica Enterprises operates two specialized Long-Term Care Skilled Nursing Facilities (SNFs) in California, uniquely positioned to serve some of the state’s most vulnerable psychiatric populations:
Vista Pacifica Enterprises operates two specialized Long-Term Care Skilled Nursing Facilities (SNFs) in California, uniquely positioned to serve some of the state’s most vulnerable psychiatric populations:
- Vista Pacifica Center is a secured, 108-bed facility specializing in geropsychiatry and Special Treatment Programs (STP). It serves adults of all ages, including those over 65, with complex psychiatric and behavioral health needs.
- Vista Pacifica Convalescent is a 49-bed residential facility providing skilled nursing care to clients with secondary psychiatric disorders, offering integrated support for both psychiatric and medical conditions.
Both facilities are designed to meet the needs of individuals who are:
- Conserved by the Public Guardian
- Gravely disabled due to serious mental illness
- Medically fragile and unable to be safely placed in lower levels of care
The demand for long-term inpatient psychiatric care with 24-hour nursing support continues to grow, particularly for older adults with co-occurring chronic medical conditions. However, access to appropriate placements remains a significant challenge, due to statewide shortages of IMD/SNF beds and challenges with finding placement in short timeframes. Vista Pacifica plays a critical role in the statewide continuum of care by accepting referrals from multiple counties, particularly for aging individuals with complex needs who cannot be served elsewhere. Without facilities like Vista Pacifica, these consumers risk remaining in inappropriate settings such as acute hospitals, or worse, homelessness.

Competitive Bid Process/Vendor Performance
On April 22, 2025, the County’s Deputy Director of GSD/Manager of Procurement/County Purchasing Agent (Purchasing Agent) approved a sole/single source procurement for these services, based on satisfactory service/reasonable prices and to avoid the interruption of County business. Without sole source approval, the County would lose the ability to provide mandated long-term inpatient psychiatric and convalescent care for County SMI clients. Factors considered, included but were not limited to: 1) Vista Pacifica provides both secure and residential facilities as well as 24-hour SNF care with a geropsychiatry focus on aging clients. Vendor also accepts some of the hardest to place older SMI consumers; 2) satisfactory performance- Vista Pacifica has been a County vendor for over five years providing competitive rates and documented successful performance in inpatient psychiatric and convalescent care; 3) competitive pricing/cost savings; and 4) County would lose the ability to place consumers in the numerous facilities operated by Vista Pacifica throughout the state. The County would be limited in its ability to identify and place consumers quickly into facilities offering an appropriate level of care, and in some cases would be unable to meet federally mandated standards regarding the provision of care for consumers.

Performance Measures associated with this Agreement are included in Attachment B. Performance data will be tracked and analyzed as specified in the Agreement. Vista Pacifica provides 24 hours skilled nursing care and clinical services in a residential setting for SMI clients with the eventual goal of a return to a community based placement. During Fiscal Year 24/25, Vista Pacifica served eight adult Yolo County clients with SMI. Only one (1) out of eight (8) clients experienced one or fewer acute psychiatric hospital stays during placement at the facility.

No general funds are required by this action. These services will be funded by 1991 Mental Health (MH) Realignment and Patient Fees. The total amount of this action is $3,140,676 for the period of July 1, 2025 through June 30, 2028. The amount of $1,046,892 is included in the HHSA recommended budget for fiscal year 2025-26. The related funding will be included in the requested budget process for future fiscal years.
