Recommended Action
- Adopt resolution authorizing the Yolo County Health and Human Services Agency to participate in and receive Transitional Housing Program (THP) – Round 7 funding in the amount of $119,904 and Housing Navigation Maintenance Program (HNMP) – Round 4 funding in the amount of $66,979 from the California Department of Housing and Community Development to provide housing services and activities to target populations in the community (Att. A. Board Resolution);
- Delegate authority and authorize the Yolo County Health and Human Services Agency Director, or designee, to execute resulting agreement in connection with the THP Round 7 and HNMP Round 4 funding, as well as execute any financial amendments to accept additional THP Allocation not to exceed $239,808, additional HNMP Allocation not to exceed $133,958, and any non-financial amendments, upon approval as to form by County Counsel; and
- Adopt budget resolution amending the Health and Human Services Agency budget for fiscal year 2025-26 to reflect an increase of $186,883 to the HHSA Operations Fund (1045), the Child, Youth & Family Child Welfare Transition Age Youth cost center (Att. B. Budget Resolution).

Reason for Recommended Action/Background
In April 2020, the Board of Supervisors adopted a resolution authorizing acceptance of the Department of Housing and Community Development ("HCD") allocation for the Transitional Housing Program ("THP") Round 1. The Board of Supervisors has continued to approve resolutions authorizing the acceptance of subsequent funding for THP Rounds 2, 3, 4, 5, and 6. On August 19, 2025, HCD issued an allocation acceptance form to Yolo County Health and Human Services Agency (“HHSA”) for THP Round 7 funding.
Funding under THP is used to identify and assist with housing services for young adults from 18 to 24 years of age in the community to secure and maintain housing (with priority given to those currently or previously in the state's foster care or probation system); improve coordination of services and linkages to community resources within the child welfare system and the Homeless Continuum of Care; and provide engagement in outreach and targeting to serve those with the most severe needs. Based on each county's percentage of total statewide number of young adults aged 18-24 years in foster care, Yolo County is currently eligible to receive an allocation of approximately $119,904 for Fiscal Year 2025-26 to enhance and continue this program.
In December 2022, the Board of Supervisors adopted a resolution authorizing acceptance of Housing Navigation Maintenance Program ("HNMP") Round 1 funding. The Board of Supervisors has continued to approve resolutions authorizing the acceptance of subsequent funding for HNMP Rounds 2 and 3.
Funding under HNMP is used to assist young adults aged 18-24 years in securing and maintaining housing (with priority access given to young adults currently or previously in the foster care or probation system), provide housing navigation services, provide housing case management, prevent young adults from becoming homeless, and improve the coordination of services and linkages to key resources across the community. Yolo County is currently eligible to receive allocation funds of approximately $66,979 for Fiscal Year 2025-26 to enhance and continue this program.
As part of this process, HCD requires that the County submit an application for the THP Round 7 and HNMP Round 4 funding in the form of Allocation Acceptance forms, accompanied by a Board Resolution authorizing the County to apply for and accept this funding, as well as designating the signatory for the associated allocation award.
Budget Amendment
On or about September 23, 2025, the Board of Supervisors for the County of Yolo adopted a budget for fiscal year 2025-26 for the Health and Human Services Agency (“HHSA”). During the fiscal year 2025-26 budget development process, HHSA had not been notified of the awarded THP Round 7 and HNMP Round 4 grant funds; therefore, the funds were not included in the adopted budget process.
Approval of the request herein will enable HHSA to add the amount of $186,883 to the Health and Human Services Agency adopted budget for fiscal year 2025-26.

Any related funding will be included in the appropriate budget process for future fiscal years.
