Recommended Action

  1. Adopt budget resolution amending the Health and Human Services Agency budget for Fiscal Year (FY) 2025-26 to reflect an increase of $27,682.07 to the Adult & Aging Criminal Justice Behavioral Health cost center; and
     
  2. Approve first amendment to Agreement No. 24-255 with Bay Area Community Services (BACS) to extend the term of the agreement through September 1, 2026, roll over $260,682 from FY2024-25 to FY2025-26 and add $500,000 to FY2025-26, resulting in a new contract maximum of $1,850,000 through September 1, 2026; update the budgets; add a service coordinator to the scope of services; and update performance measures for the provision of Proposition 47 supportive housing Connections to CARE (Community Assistance, Recovery, and Empowerment) program services. Reason for Recommended Action/Background

Budget Amendment
On or about September 23, 2025, the Board of Supervisors for the County of Yolo adopted a budget for fiscal year 2025-26 for the Health and Human Services Agency (“HHSA”). During the budget development process, the need to include additional Proposition 47 funding for fiscal year 2025-26 was not identified; therefore, the funds were not included in the adopted budget process.

For reference, on or about November 22, 2022, HHSA entered into a revenue agreement with the California Board of State and Community Corrections (agreement number BSCC 556-22), to participate in the Proposition 47 grant funding for people involved in the criminal justice system who have mental health and/or substance use disorders.

Approval of the request herein will enable HHSA to add the amount of $27,682 to the Health and Human Services Agency adopted budget for fiscal year 2025-26.

Contract Amendment
Proposition 47 Grant funds target a reduction in recidivism by expanding access to substance use disorder (SUD) treatment and trauma-informed intensive case management, peer support, housing, and other wraparound supportive services for adults with a history of SUD and current or recent justice system involvement, with an emphasis on individuals experiencing homelessness. The Connections to Care program aims to:
  • Improve the behavioral health of participants with identified behavioral health needs through engagement in appropriate services and reduced risk/harm related to ongoing substance use.
  • Increase the self-sufficiency of participants through stable housing, improved income, and enrollment in health insurance; and
  • Reduce justice system involvement by preventing further involvement and providing diversion opportunities, allowing participants to have their SUD-related charges dismissed.
The target population of the Connections to Care program includes adults with a history of SUD or co-occurring disorders (COD) who have had recent involvement (most recent within three years) with the justice system, with an emphasis on individuals experiencing homelessness. Due to a large portion of the population being unhoused and the intensive case management provider having unexpended funds, we are requesting an additional $500,000 be added to the housing contract to ensure participants have a safe place to focus on recovery.

Bay Area Community Services (BACS) has become a leader in homeless prevention programs. With approval of this amendment, BACS will continue providing case management to Proposition 47 Connections to CARE program clients, along with an array of housing services, including:
  • Housing Navigation Services
  • Homeless Prevention Services
  • Housing Stabilization Services
  • Rental Assistance
  • Supportive Services, including landlord relationship-building
  • Interim Housing Support
  • Short-Term Residential Support
  • Mental Health Support
  • Rapid Re-Housing and Emergency Housing Support
  • Affordable Housing
Additionally, the amendment adds an employment services specialist to ensure participants have access to guidance and support in obtaining employment.

On or about September 10, 2024, the County and BACS entered into an agreement for the provision of Proposition 47 supportive housing Connections to CARE program services. HHSA staff requests approval of this First Amendment to the agreement to provide stable housing to all participants and add an employment specialist who will assist and guide participants in obtaining employment. These additional services will support the continued success of Proposition 47 – Connections to Care Program.

Performance Measures included in this Agreement are as follows:
HHSA confirms BACS is performing satisfactorily under this Agreement. Performance measures were originally included in this Agreement and are being revised at this time (See Att. C. Performance Measures). In fiscal year 2024-25, BACS received 37 referrals and 78% of the households received rapid rehousing services and secured temporary stable housing.

Competitive Bid Process/Vendor Performance

These services were initially procured via Request for Proposals (RFP) #HEALRFPDL2204. However, a year into providing services, the awarded provider terminated that agreement.

On March 25, 2024, the Yolo County Deputy Director/Manager of Procurement approved a sole source procurement for a continuation of these services with BACS. Factors considered included but were not limited to: 1) There were no other service providers that were willing to provide these services; 2) the service cost is fair, and a specific amount is allocated from the state specifically for housing services; and 3) BACS has the ability to build housing inventory in the County for our Connections to CARE program.

Without sole source approval and the approval of the agreement with BACS, the County’s ability to meet its grant requirements would be significantly hindered and may require the County to return grant funds to the State.

No general funds are required for this action. These services will be funded by Proposition 47 (Prop 47) funds. This amendment rolls over $260,682 of unexpended funding from FY2024-25 to FY2025-26 and adds an additional $500,000 to address FY2025–26 program needs. This action increases the contract maximum to $1,850,000 for the period from the effective date through September 1, 2026. As outlined in the attached budget resolution, additional expenditure authority in the amount of $27,682.07 is requested to fully fund the recommended action. The rolled over funds and the additional funds of $472,318 are included in the FY2025-26 adopted budget.
