Reason for Recommended Action/Background
On or about June 8, 2021 the County entered into an Agreement with CommuniCare Health Centers to provide school based behavioral healthcare, physical healthcare and social services to students at Woodland Community College. The initial term of the Agreement was from January 19, 2021 through June 30, 2023 with a contract maximum of $449,999.50.
The school based physical healthcare, behavioral healthcare, and related social services are being provided in a client-centered, culturally, and linguistically competent manner to improve beneficiary health and well-being of Woodland Community College (WCC) students, with a sensitivity to providing Spanish-speaking and Russian Speaking services. Services have taken place at the three (3) Woodland Community College campuses, located in Woodland, Williams and Clearlake, as well as CommuniCare’s three (3) primary care service locations, located in Woodland, West Sacramento and Davis.
On or about July 20, 2021, the Parties amended the agreement to update language regarding terms of payment.
HHSA requests approval of this Second Amendment which adds funding in the amount of $60,000.64 for Fiscal Year 2022-23 for a new contract maximum of $510,0004.14 and updates and clarifies language in the Agreement. HHSA and Yuba Community College District (YCCD) are in partnership to provide the physical, behavioral and social services on the community college campuses. In partnership, YCCD contributes $60,000 per fiscal year to these services. Increasing the budget for these contracted services reflects HHSA and YCCD’s contributions toward the services.
Performance Measures included in this Agreement are as follows:
The Performance Measures were included in the underlying Agreement and are not being revised at this time (See Att. B. Performance Measures)
HHSA confirms CommuniCare Health Centers, Inc. has performed satisfactorily under this agreement. CommuniCare has submitted quarterly performance measures timely, reflecting 100+ students per quarter are receiving physical and behavioral health services on the college campuses. CommuniCare has participated in health and resource fairs and tabling events to conduct outreach campuswide. YCCD states their appreciation for the on-campus services and is working with HHSA to create program expansion to serve more students with these essential services.

Competitive Bid Process/Vendor Performance
On September 24, 2020, Yolo County Health and Human Services Agency (HHSA) issued a Request for Proposals (RFP) from qualified community-based healthcare providers to provide physical healthcare, behavioral healthcare and related social services to college students at the Woodland Community College. The RFP was open for 41 of days, just under 6 weeks, and staff offered potential bidders an opportunity to ask questions during a Mandatory Bidders’ Conference on October 7, 2020. Bidders were offered the opportunity to submit proposals by dropping off a paper proposal directly at HHSA.
HHSA received one proposal(s) from this solicitation. The proposal submitted by CommuniCare Health Centers was selected as the top choice by all members of the Review Committee, and was evaluated and determined to meet all the required qualifications necessary to successfully provide these services.
