Reason for Recommended Action/Background
Proposition 47 Grant funds target a reduction in recidivism by expanding access to substance use disorder (SUD) treatment and trauma-informed intensive case management, peer support, housing, and other wrap-around supportive services for adults with a history of SUD and current or recent justice system involvement, with an emphasis on individuals experiencing homelessness. The project aims to improve the behavioral health of participants with identified behavioral health needs through engagement in appropriate services and reduced risk/harm related to ongoing substance use; increase the self-sufficiency of participants through secured stable housing, improved income, and enrollment in health insurance; and reduce the justice system involvement for participants through prevention of further justice system involvement and diversion in which participants may have their SUD-related charge dismissed.
Target population includes adults with a history of SUD or Co-occurring Disorders (COD) who have (1) been arrested for low-level drug possession or public intoxication and are eligible for Harm Reduction Diversion or (2) been arrested in the prior three years and either entered Yolo County’s Crisis Now receiving/sobering center with a SUD-related crisis through pre-arrest contact with specialized law enforcement units or were identified directly by homeless outreach teams. As a wraparound program with housing support, the target population includes individuals who are also experiencing homelessness. This population was identified due to their justice system overrepresentation and evidence of unmet BH and housing needs.
The components of the project that will be operated by Community-Based Partners include:

Competitive Bid Process/Vendor Performance
On October 19, 2022, the County via the Department of General Services, Procurement Division (DGS-Procurement) [formerly Department of Financial Services, Procurement Division] issued a Request for Proposals (RFP) on behalf of the Health and Human Services Agency (HHSA) for Proposition 47 Case Management, Civil Legal and Housing Services. The solicitation was open for 33 days, (just over 4 weeks). On October 25, 2022, DGS and HHSA offered potential bidders an opportunity to ask questions during a bidder’s conference.
The County received two (2) proposal(s) from this solicitation. DGS reviewed the proposal for minimum requirements and two (2) proposals were moved forward to the Review Committee.
The Review Committee evaluated the proposals and determined that the proposal submitted by CommuniCare Health Centers met all the required qualifications necessary to successfully provide these services. On January 17, 2023, the County issued an Intent to Award letter.

No general funds are required by this action. These services will be funded by BSCC Grant Funds. The total amount of this action is $2,797,389 for the period of July 1, 2023 through March 1, 2026. The amount of $964,417 is included in the HHSA adopted budget for fiscal year 2023-24. The related funding will be included in the requested budget process for future fiscal years.
