Approve first amendment to memorandum of understanding with Mercy New Hope, L.P. dba Mercy Housing Management Group to update the unit set-asides and the homeless and disability requirements which oversee the delivery and coordination of the housing project known as West Beamer Place in Woodland, with a zero-dollar amount for the period beginning October 23, 2018. The initial term of this MOU began October 23, 2018 and will automatically renew for additional twelve (12) month terms unless either party provides thirty (30) days’ prior written notice of non-renewal. (No general fund impact) (Sullivan)

Recommended Action

  1. Approve the first amendment to memorandum of understanding with Mercy New Hope, L.P. dba Mercy Housing Management Group to update the unit set-asides and the homeless and disability requirements which oversee the delivery and coordination of the housing project known as West Beamer Place in Woodland, with a zero-dollar amount for the period beginning October 23, 2018. The initial term of this MOU began October 23, 2018 and will automatically renew for additional twelve (12) month terms unless either party provides thirty (30) days’ prior written notice of non-renewal; and
     
  2. Delegate authority to the Director of the Yolo County Health and Human Services Agency, or designee, to execute any non-financial amendments of this MOU, upon approval as to form by the County Counsel.

Reason for Recommended Action/Background

Yolo County CalWORKs Housing Support Programming allows for the expenditure of State CalWORKs funding for permanent supportive housing for families who are homeless and meet program eligibility.  Supportive housing is a nationally recognized evidence-based practice that has consistently shown that coupling appropriate services with permanent housing increases housing stability and the likelihood that the tenant will remain connected to treatment and ultimately self-sufficiency.

The County previously submitted a collaborative application requesting capital development and operating subsidies to the State Department of Social Services for the development of the permanent supportive housing project at Beamer Street in Woodland for homeless families with mental illness.  Included are families facing housing insecurity that are CalWORKs and eligible for the Housing Support Program.  These families could be those involved with Child Welfare Services (Linkages) and accessing funding through Bringing Families Home.  Intensive case management and supportive services are tied to these families to support barrier reduction and assisting movement toward self-sufficiency.

The Memorandum of Understanding entered into in October 2018 (“MOU”) established a partnership between the County and Mercy Housing Management Group (“Mercy”) to provide seamless and high-quality services to each tenant of the housing project, based on the families’ individual needs.  The County and Mercy coordinated their resources toward the common goal of enabling persons with serious mental illness who are homeless, or at risk of homelessness, and their families to achieve permanent supportive housing and self-sufficiency. This was accomplished by promoting the integration of affordable housing and appropriate supportive services, including mental health support systems.

By approving this first amendment to the MOU, which adjusts language to Exhibit 1 – Scope of Services, the County and Mercy will continue their partnership to provide a range of services for homeless families with mental illness.

HHSA confirms Mercy New Hope, L.P. has performed satisfactorily under this MOU. The MOU does not contain an exhibit outlining requested performance measure data; however, the MOU contains a section outlining objectives for informal data reporting.  The informal objectives are separated into two categories: Outcome Objectives and Service Objectives and include the following metrics:
 
Outcome Objectives
  1. CalWORKs Housing Support Program (“HSP”) has referred twenty-nine families to West Beamer Place (“WBP”) to fill the twelve (12) units allocated to HSP; 93.1% of the families retained housing for at least one year.
  2. 100% of CalWORKs HSP families receive Medi-Cal as part of their benefits. They were receiving or were referred to Mental Health Services/Substance Use Services (MH/SAS) at the time of their referral to WBP. 
  3. Support services are offered to all families, including formerly homeless or foster youth families, as part of Intensive Case Management (“ICM”).
  4. ICM staff and WBP staff work collaboratively to offer an array of services to all resident families.
Service Objectives
  1. ICM staff work with all HSP families to identify barriers to permanent housing and offer referrals to services that will help mitigate the barriers.
  2. All CalWORKs HSP families are assigned to an ICM who will meet with them at least once a month. Meetings may occur more frequently depending on the needs of the family.
  3. WBP Property Management (“PM”) staff and embedded ICM staff have developed a strong partnership and work hand in hand to address emerging concerns via the use of housing support plans.  The plans are designed to aid families in mitigating issues that place their housing at risk. The flow of information between PM staff, ICM staff, and the family is seamless and ensures everyone stays informed.  The use of Housing Support Plans facilitates the referral process to needed services that help families retain their housing. 
  4. WPB delivers a monthly calendar of services available to all residents; the calendar is shared with service providers and is posted in public areas.
  5. WBP administers surveys to all residents in Spring and Summer. The data collected from the surveys is utilized to improve services for families.
  6. WBP PM has recurring weekly service provider meetings to discuss family concerns and a weekly meeting with case management staff to discuss major upcoming events for subsequent dissemination to families. These events include mandated state inspections.
