Approve and authorize the Chair to sign Agreement No. 24-0046 with the California Department of Health Care Services for the period of January 1, 2024 through June 30, 2027 for the transfer of public funds to provide support for the non-federal share of risk-based payments to managed care health plans for Medi-Cal health care services.

Reason for Recommended Action/Background

Each year, the Yolo County (County) Health and Human Services Agency (HHSA) seeks to secure federal financial support for services delivered to the Medi-Cal population that have not previously received federal matching funds. The mechanism for securing these funds involves a rate range intergovernmental transfer (IGT) process pursuant to sections 14164 and 14301.4 of the Welfare and Institutions Code through an agreement with the California Department of Health Care Services (DHCS).  The rate range IGT process requires that County transfer local funding to DHCS, who uses the funds to draw down additional federal matching funds.  DHCS then transfers the original County funds, along with the federal matching funds (minus an administrative fee), to the appropriate local Medi-Cal Health Plan. Yolo County's Medi-Cal Health Plans are Partnership Health Plan of California (Partnership) and Kaiser Foundation Health Plan (Kaiser). The local Medi-Cal Health Plans then awards these funds to back to their partnering entities, including the County.

The estimated amount of rate range IGT funding to be transferred to DHCS is $2,870,854 for the period of January 1, 2024 through December 31, 2024.  A true-up process will involve a review of the State Medicaid rate range approximately two years after the matching funds are transferred from County to State. This true-up process will result in either additional matching funds being due from County or previously transferred matching funds being refunded back.
 
HHSA is requesting that the Board of Supervisors approve and authorize the Chair to sign this Agreement to receive rate range IGT funds to assist in funding Medi-Cal services to eligible members of the community.

No general funds are required by this action. The $2,870,854 that HHSA will transfer to DHCS will be returned to HHSA, along with the federal financial participation (less the 20% administrative fee) resulting in revenue generation for the County.
