Recommended Action

Approve eleventh amendment to Agreement No. PO 3646 with Vista Pacifica Enterprises to add funding in the amount of $90,000, for a new contract maximum of $3,981,892, for the period of July 1, 2020 through June 30, 2025, for the provision of 24-hour supervised clinical care to adults with severe mental illness (SMI).

Reason for Recommended Action/Background

As the local Mental Health Plan (MHP) for Specialty Mental Health Services, the County is mandated to provide consistent and quality care in secure residential settings, when deemed necessary, for the safety and stability of individuals with SMI. Vista Pacifica Center in Riverside operates an Institute for Mental Disease (IMD) program, providing treatment and care to Yolo County SMI beneficiaries since 2016.  The program offers specialized treatment including pharmacological, therapeutic, and rehabilitative interventions in a safe and secure environment. On or about July 1, 2020, the County entered into an agreement with Vista Pacifica to provide 24-hour care, non-state hospital locked setting to referred adults who are conservatees of Yolo County Public Guardian and require treatment in such a secure environment (the Agreement).  Since 2020, the Agreement has been amended several times to meet various business needs, mainly to increase the contract amounts to align with utilization and to revise rates.

HHSA staff requests approval of this eleventh amendment due to higher than anticipated utilization of services under the contract. This amendment adds $90,000 in funding for FY 2024-25, increasing the total contract maximum to $3,981,892. The additional funds are necessary to cover the remaining FY2024-25 invoices.

Competitive Bid Process/Vendor Performance

On June 30, 2020, the then acting County’s Deputy Director of GSD/Manager of Procurement/County Purchasing Agent (Purchasing Agent) approved a sole/single source procurement for these services, based on satisfactory service/reasonable prices and to avoid the interruption of County business. Yolo County Procurement Manager approved a sole source procurement for these services. Without sole source approval, the County would have lost the ability to provide mandated long-term inpatient psychiatric and convalescent care for County referred mental health clients. Factors considered, included but were not limited to: 1) Vista Pacifica provides both secure and residential facilities as well as 24-hour skilled nursing facility care with a geropsychiatry focus on aging clients; 2)Vendor accepts some of the hardest to place older SMI consumers; 3) satisfactory performance- Vista Pacifica has been a County vendor for over five years providing competitive rates and documented successful performance in inpatient psychiatric and convalescent care; 4) competitive pricing/cost savings; and 5) County would lose the ability to place consumers in the numerous facilities operated by Vista Pacifica throughout the state. The County would be limited in its ability to identify and place consumers quickly into facilities offering an appropriate level of care, and in some cases would be unable to meet federally mandated standards regarding the provision of care for consumers.

HHSA confirms Vista Pacifica has performed satisfactorily under this agreement as they provided services to Yolo County residents during FY 2024-25 with a total of 1905 bed days utilized as of the end of March 2025. One hundred percent (100%) of the County placed clients experience one or fewer acute psychiatric hospitals during the placement at the facility in the last year.

No general funds are required by this action. These services are funded by Mental Health 1991 Realignment (MH 1991 Realignment). The action increases the contract maximum by $90,000, bringing the total contract amount to $3,981,892 for the period of July 1, 2020 through June 30, 2025. The additional $90,000 is included in the HHSA budget for fiscal year 2024-25, and the source of the additional funds is MH 1991 Realignment.
