Recommended Action
- Approve agreement with Foothill Heights Care Center, LLC in the amount of $201,000 for the period of August 1, 2025 through June 30, 2026 for the provision of 24-hour supervised clinical and nursing care services to long-term consumers with severe mental illness (SMI); and
- Delegate authority to the Director of the Yolo County Health and Human Services Agency, or designee, to: 1) exercise optional year extensions for fiscal years 2026-27 and 2027-28 and 2) issue related option year notices. See Section B of Att. A.

Reason for Recommended Action/Background
As the local Mental Health Plan (MHP) for Specialty Mental Health Services, the County is mandated to provide consistent and quality care in secure residential settings, when deemed necessary, for the safety and stability of individuals with Severe Mental Illness (SMI).
Foothill Heights Care Center provides a residential Skilled Nursing Facility (SNF) for SMI adults. This facility provides continuous care, supervision, and monitoring of a client's physical, mental, emotional, and social functioning for hard-to-place clients with behavioral problems. Foothill Heights Care Center is willing to accept behaviorally challenged clients languishing in inpatient hospital settings who are no longer acute but cannot be placed.
Health and Human Services Agency staff requests approval of this agreement to ensure continued access to essential services for a population with limited placement options and significant clinical needs. It supports our commitment to providing the appropriate standard of care and protecting the safety and well-being of our most vulnerable clients.

Competitive Bid Process/Vendor Performance
On June 13, 2025, the County’s Deputy Director of GSD/Manager of Procurement/County Purchasing Agent (Purchasing Agent) approved a sole/single source procurement for these services, based on a determination that the proposed costs are fair, reasonable, and consistent with the cost of other social rehabilitation programs across the state. Furthermore, Foothill Heights Care Center has demonstrated the capacity and willingness to serve clients who are behaviorally challenged and languishing in inpatient hospital settings despite no longer having acute needs, but who are still difficult to place. Without sole source approval, the County would be limited in its ability to identify and place clients quickly into facilities offering this appropriate level of care, and in some cases would be unable to meet federally mandated standards regarding the provision of care for consumers. Factors considered, included but were not limited to: 1) Their programs are designed to attain the highest practicable physical, mental and psychosocial well-being of each resident in a timely and cost-effective manner;2) There is no other program that offers the same type of pre-admit engagement in preparation of residential programming for adults; 3) Foothill Heights Care Center is also equipped to divert higher needs clientele from state hospitals with high level of support structured into their varied campuses.

No general funds are required for this action. These services will be funded by 1991 Mental Health Realignment. The total amount of this action is $201,000 for the period of August 1, 2025 through June 30, 2026. The amount of $201,000 was included in the HHSA adopted budget for FY2025-26. The related funding will be included in the requested budget process for future fiscal years.

In no event shall the term of the Agreement extend beyond June 30, 2028, nor shall the total contract maximum exceed the amount of SIX HUNDRED THIRTY-NINE THOUSAND DOLLARS ($639,000), unless otherwise agreed to in writing by the parties and in conformity with the then-current County Procurement Policy.
