Reason for Recommended Action/Background
The County entered into an agreement with the California Department of Health Care Services (DHCS) to provide Drug Medi-Cal Organized Delivery System (DMC-ODS) Substance Use Disorder (SUD) treatment services to eligible clients. DMC-ODS provides SUD treatment services to Medi-Cal eligible individuals with substance use disorders residing in a county that has elected to participate in DMC-ODS. The program provides members with access to care and system interaction needed to achieve sustainable recovery.
SUD Continuum of Care services include the following:
SUD Continuum of Care services include the following:
- Residential treatment services
- Outpatient and intensive outpatient treatment services
- Residential withdrawal management (detoxification) services
- Clinician Consultation
- Recovery Residences
- Narcotic treatment program services/opioid treatment program
These services are provided to any eligible Yolo County resident referred through an access point established by Yolo County or directly requesting services through the provider and include, but are not limited to, the following populations/programs:
- CalWORKS Participants
- Individuals with an open Child Welfare Services (CWS) Case Plan
- Individuals who are pregnant or postpartum (Perinatal Services)
- Individuals who are involved with the criminal justice and those partially funded by system (AB109)
- Medically Indigent Adults
- Individuals with co-occurring substance use and mental health disorders and substance-use disorders.
On or about December 14, 2023, the County entered into an Agreement with Yolo Wayfarer Center with a term of January 1, 2024, through June 30, 2024, which included five (5) optional extensions through FY2028-29. The Agreement has been amended, and optional extensions executed to allow services to be continued through June 30, 2026.
HHSA is requesting approval of the third amendment to add funding in the amount of $85,000 for 2025-26 which will allow HHSA to extend services to include women’s recovery residence services at a new service location: 913 Court Street in Woodland, CA.
The amendment also includes updates to the rates effective July 1, 2025. When DHCS increases their rates, HHSA elects to pass through these rate increases to our contracted network providers. Per Minute Order 25-35, dated May 20, 2025, the Board approved the HHSA Director to sign amendment(s) for rate increases as long as the increase did not exceed four percent (4%). This rate increase per this Amendment is under the 4% threshold; increasing 2024-2025 rates by 3.09%. Please see (Att. A. Third Amendment) to review the rate tables.

Competitive Bid Process/Vendor Performance
On March 3, 2023, the County via the Department of General Services, Procurement Division (DGS-Procurement) issued a Request for Proposal (RFP) on behalf of the Health and Human Services Agency (HHSA) for Substance Use Disorder Adult Continuum of Care services. The solicitation was open for 64 days, (just over 9 weeks). On March 24, 2023, the County offered potential bidders an opportunity to ask questions during a bidder’s conference.
The County received three (3) proposal(s) from this solicitation. DGS reviewed the proposals for minimum requirements and all three (3) proposals were moved forward to the Review Committee, comprised of two (2) HHSA staff members, one (1) Yolo County Probation staff member, and one (1) Placer County staff member.

HHSA confirms that the Yolo Wayfarer Center is performing satisfactorily under this Agreement. Performance Measures were originally included in the Original Agreement and are not being revised at this time (see Attachment B-Performance Measures).
In FY 2023–24, 234 clients were served, with a completion rate of 40%. In FY 2024–25, a total of 265 clients received services. During this period, 95% of clients successfully completed their treatment plans.

No general funds are required for this action. The additional funds are funded by Medication Assisted Treatment (MAT) Grant. The action increases FY 2025-26 funding by $85,000 for a revised FY 2025-26 funding amount of $1,725,000 and a new contract maximum of $4,115,000 for the period of January 1, 2024 through June 30, 2026. The amount of $85,000 was included in the HHSA adopted budget for fiscal year 2025-26. HHSA is not requesting funding increases for optional terms at this time, those amounts will be determined based on future contract needs. The related funding will be included in the requested budget process for future fiscal years.
