Recommended Action

Approve a lease agreement with Communicare+OLE for use of County facilities at 1038 Triangle Court (500B Jefferson Blvd.) in West Sacramento.

Reason for Recommended Action/Background

The County owns the property and structures at 1038 Triangle Court, West Sacramento, which is commonly referred to as 500B Jefferson Boulevard.  This is one of two properties that form the West Sacramento County Service Centers campus.  500B Jefferson is a 26,530 square foot building that contains Communicare medical offices and the County's Health & Human Services Agency Mental Health Clinic.  Communicare has leased a portion of 500B Jefferson since 2005.  The lease expired at the end of March 2026 and staff negotiated since December 2025 for a new lease.  During this negotiation, the prior lease was extended by one month through April 2026 to allow negotiations to finish.  This proposed lease will be for the period of May 1, 2026 through April 30, 2030, with an option to extend by one additional year.  During this time, Communicare is working with the City of West Sacramento on an alternative location for the medical offices.  If sufficient progress is made on the relocation, an additional year may be added to the lease term.

This lease proposes a four-year period to migrate the existing dollar per square foot ($/sf) rate to market rate.  All revenue from use of County facilities is deposited in the County's General Fund.  Currently, Communicare pays $5,880.18 per month in rent.  The proposed lease will increase the base rent to $16,432.03 per month effective in the first year of the new lease, escalating annually thereafter to $32,699.74 per month in year 4. If the parties agree to further extend the lease, the rent will be subject to additional 3% increases.

Collaborations (including Board advisory groups and external partner agencies)

General Services collaborated with the Health & Human Services Agency, the County Administrator's Office, and the County Counsel's Office.  The Board of Supervisors held closed sessions regarding the lease negotiations on January 13, 2026 and March 3, 2026.

As discussed previously, the existing amount of the lease is $5,880.18 per month.  The new amount is $16,432.03 per month for the first year of the agreement.  This increase will bring in unanticipated revenue of $21,104 for the balance of Fiscal Year 2026.  The Base Monthly rent during the term of this agreement can be found below:
